Supported Living Services
Our specialist supported living services provide individually tailored packages of support that enable people with a range of disabilities and complex needs, including mental health needs, to live independent lifestyles of their choosing including access to education and employment.
Working in partnership with housing providers we provide a range of innovative housing solutions including shared housing, studio flats and self contained flats that are designed to reflect what is most suited to an individual’s needs and wishes. Where required, our services have been adapted to meet specific sensory and physical needs.
Our specialist staff teams include dedicated Tenant Liaison Officers who support tenants to maintain their own tenancies.
Our services promote greater choice, control and independence.
Domiciliary Care
We fully understand the importance of maintaining independence and exercising choice and control. Our domiciliary care services fully support people who live in their own homes to follow lives of their choosing as independently as possible.
Our domiciliary services range from providing support to both learn and assist with domestic tasks such as cooking, household cleaning, home repairs, organising benefits and paying bills to personal tasks such as washing, dressing and continence management.
We aim to gradually reduce the level of support we provide as confidence increases and daily living skills are developed.
Support in the Community and Day Skills
Using our person centred approach we support individuals to become active and contributing members of their local communities. The type, level and duration of the support we provide is highly flexible and varies from a couple of hours a week to more intensive daily support. The services can be delivered on an individual or group basis.
We are chosen to provide support by those who want to access their local communities from their own flats or whilst living with families or carers. Support varies significantly from helping with weekly shopping to attending local appointments. We also support a significant number of people to access social, leisure, educational or employment opportunities of their choosing.
Residential Adult Services
Over the last year many of those who have chosen us to provide active support have successfully moved to greater independence in supported living.
Our services all support service users through our Person Centred Active Support approach. They provide highly personalised, safe and fun environments where service users can develop their social, communication and life skills with a view to gaining greater levels of independence, including education and employment.
Our services are specifically adapted as required to meet any additional needs such as limited mobility, physical disabilities and complex healthcare conditions. Where appropriate we invest in Assistive Technology (AT), specialised equipment and communication aids to maintain and improve independence.
Using Person Centred Planning we support choice and access to social, educational and employment activities. People are encouraged to meaningfully contribute to the day to day running of their services including recruiting staff.
Residential Children's Services
We provide specialist care and therapy to children with learning disabilities and complex physical and health needs, mental health needs and Autistic Spectrum Disorders in services that are homely, safe, fun and specially adapted to meet the individual needs of the children who live there.
Using Person Centred Planning and a multidisciplinary approach each child has a stimulating daily programme of activities that includes accessing the national curriculum at local schools and colleges.
Daily programmes are designed to encourage personal development, increase self confidence and facilitate transition into more independent lives.
