CMG Wheel of Excellence

Our person centred approach ensures that the support we provide remains individual to the needs and aspirations of our service users whilst measuring progress by working towards a set of realistic outcomes.

CMG has developed 16 outcome measures with the Department of Health, which are shown in our “Wheel of Excellence”. This measures service users’ progress in relation to independence, relationships, health and wellbeing.

The “Wheel of Excellence” is completed every six months with each service user supported by CMG and a visual progress chart is produced which enables the service users, staff, care managers and family members to see progress over time:
