- With learning disabilities, physical disabilities, communication and sensory impairments and associated physical and health needs
- Aged 18 and over
Services
- Person centred planning
- Individualised health action planning
- A unique approach using assistive technology
- Support with life and independence skills development
- Support to identify and maintain education placements and/or employment
- Support to identify and attend social and leisure activities
- Up to 24 hour staffing
- Waking night staff
