Our team at this service supports people:

• With learning disabilities, physical disabilities, communication and sensory impairments and complex health needs including epilepsy
• In transition aged 18 and over

Services

• Person centred planning
• Support with life and independence skills development
• Support to identify and maintain education placements and/or employment
• Support to identify and attend social and leisure activities
• Fully adapted environment including assistive technology
• Up to 24 hour staffing
• Waking night staff
