The initial assessment of need is hugely important. We work closely with the referring organisation so that we can match the assessment of need with a service we provide.

Each of our four regions are set up to achieve this via people who are local both to the potential service user and the resource that may be able to meet that need.

Our managers, whether domiciliary care, supported living or residential, assist in the process of matching need to service provision and our referrals team work closely with the service user and their circle of support to achieve this.

Our individual support plans identify clear objectives and anticipated outcomes. The support plan is a joint document which is shared with the commissioning authority, the service user, their family and all the people involved in their support and development. This is reviewed on a regular basis to meet changing need. Where appropriate this will be reflected in the fees we charge.
