The views and experiences of everyone we support and those of their families, carers and friends are at the heart of everything that we do.

Everyone we support is assisted to develop a Person Centred Plan (PCP) that focuses on enabling each person to achieve their individual dreams and aspirations. These may include making friends, achieving increased independence and gaining education and/or employment.

Service users are supported by our teams to develop lifestyles of their choosing through our Person Centred Active Support (PCAS) approach. Our dedicated PCP and PCAS Co-ordinator is on hand to ensure that our teams and everyone they support continue to develop and achieve better outcomes.

We strike a balance between appropriate safeguarding whilst providing support to exercise full choice and control. We do this by developing individual risk strategies and where appropriate include support from our dedicated Placement Support and Quality Support Teams. An important part of supporting service users to make informed choices about their lives is supporting them to understand both the benefits and the risks those choices might include.

We understand the importance of being able to demonstrate clearly the progress being made by everyone we support on a regular basis. This is not only important to those who commission our services but critical to our service users and their families. We have developed an outcomes framework with the Department of Health called the Wheel of Excellence. The Wheel of Excellence ensures that we are constantly improving and provides evidence of our commitment to achieving excellence in our services for everyone we support.

We also use a framework that evidences the progress being made in differing formats that are appropriate to make informed choices about future support needs.
